What to Know for Your
First Appointment

We want you the be well prepared for your first visit with us. Please review the following information before your visit. 

Payment Options 

We accept a wide range of payment options, including cash, credit card, money orders, and personal checks. We also accept purchase orders from reputable businesses by prior written agreement and file worker’s compensation claims on a case-by-case basis.

All payments are expected in full at the time of service, unless advance arrangements are made. 

Accepted Insurances

We strive to make high-quality healthcare available to all. We accept all major insurances and will ask you to present your insurance cards before each visit. We will submit to your primary and secondary insurance. If you have a co-pay or are compensated directly by your insurance, we will charge at the time of service. 

Discounted Services 

We offer a 20% discount for full payment at the time of service. You may qualify for the Sliding Fee Scale Discount that we offer to all patients, which is based on Federal Guidelines for family size and income. To receive this discount, you will be required to fill out an income declaration that determines the amount you owe and your household size. Additional income verification may be required. 

Translation Services 

We provide translation services available in up to 40 different languages. 

Extended Hours 

We provide evening hours for your convenience on Wednesdays, with appointments available until 7pm. We are on call 24/7 in case of an emergency. 

COVID Policy 

Masks are required when visiting Cama’i Community Health Clinic. We provide COVID-19 vaccines and testing.
